These forms should be filled out by the member and/or their parents with a copy given to the club director or coach. The coach is required to have these forms with them at all times in case of a medical emergency.

Lystedt Concussion Waiver Form

The "Zackery Lystedt Law" regarding head injury policies in youth sports was enacted in 2009 to encourage greater awareness and safety for youth athletes. It requires certain notifications be given to athletes, parents and coaches. It also creates policies for how youth athletes should be treated when suspected of having a head injury. In accordance with this state law, all junior club volleyball players must sign this agreement in order to participate in PSR junior volleyball.
